FoM Grant Application Cover Sheet (MS Word)
This form is required to be submitted to FoM with all grant and award applications.
FoM Request for Dean’s Approval of Salary Award Applications (PDF)
This form required to be submitted to FoM with all Salary Awards – ensures that the Department Head or School Director has reviewed the application before being submitted to the FoM.
ORS Sponsored Research Information Form (SRIF) (PDF)
This form is required to be submitted to FoM when funds have been received that were unsolicited, or a copy of the proposal is not available. When a SRIF is submitted, the FoM cover sheet is not required.
SPARC Internal Review Application Form (MS Word)
This form, plus a ½-1 page description of the proposed research project, must be submittted to SPARC to set up Internal Review.
SPARC Internal Reviewer Sign-Off Form (MS Word)
This form is required to confirm that an applicant has fulfilled Internal Review (through SPARC)
